Rifampin-resistant Neisseria meningitidis

چکیده
To The Editor: Immediate management of meningococcal disease requires antimicrobial drug treatment of patients with β-lactams and chemo-prophylaxis of contact persons with rifampin. High-level resistance to rifampin (MIC >32 mg/L) in Neisseria meningitidis is provoked by mutations (most frequently at the residue His 552) in the rpoB gene encoding the β subunit of RNA poly-merase (1,2). Resistance may lead to chemoprophylaxis failure and must be rapidly detected (3). Concerns have been raised about the clonal spread of resistant isolates (1); however, rifampin-resistant isolates are rarely reported. We tested 6 N. meningitidis isolates corresponding to 3 pairs of linked cases of meningococcal disease. In each pair, the index case was due to a rifampin-susceptible isolate and was followed by the secondary case due to a resistant isolate in a contact person. Phenotyping and geno-typing of the isolates showed that each pair belonged to a different major serogroup (A, B, and C) and to a different genetic lineage (ST-7, ST-32, and ST-2794) (Figure). We next amplified a fragment in rpoB between codons 421 and 701 by using oligonu-cleotide rpoBF1

Rifampin resistance in Neisseria meningitidis due to alterations in membrane permeability.
Rifampin-resistant (Rifr) Neisseria meningitidis strains are known to have single point mutations in the central conserved regions of the rpoB gene. We have demonstrated two distinct resistance phenotypes in strains with identical mutations in this region, an intermediate level of resistance in Rifr clinical isolates and a high level of resistance in mutants selected in vitro. The possible role...
متن کاملSerogroups and antibiotic susceptibility patterns of Neisseria meningitidis isolated from army recruits in a training camp.
Invasive Neisseria meningitidis infection is rare but carries a high mortality rate. The carriage rate in the normal population is around 10% and can be higher in confined populations. A study on the prevalence of carriage of N. meningitidis was conducted among 3195 army recruits after 2 months of intensive training in an army camp. N. meningitidis was isolated from 37.0% of these recruits. Two...
متن کاملMeropenem susceptibility of Neisseria meningitidis and Streptococcus pneumoniae from meningitis patients in The Netherlands.
In-vitro susceptibility of 299 Neisseria meningitidis and 157 Streptococcus pneumoniae strains from meningitis patients in The Netherlands in 1993 and 1994 to meropenem was determined using the Etest. Susceptibility to penicillin, ceftriaxone, and chloramphenicol was also determined. Rifampicin susceptibility was additionally tested for N. meningitidis. Of the meningococci, 4.3% were of interme...
متن کاملPopulation-Based Surveillance of Neisseria meningitidis Antimicrobial Resistance in the United States
Background. Antimicrobial treatment and chemoprophylaxis of patients and their close contacts is critical to reduce the morbidity and mortality and prevent secondary cases of meningococcal disease. Through the 1990's, the prevalence of antimicrobial resistance to commonly used antimicrobials among Neisseria meningitidis was low in the United States. Susceptibility testing was performed to asce...
متن کاملDescription of new mutations in the rpoB gene in rifampicin-resistant Neisseria meningitidis selected in vitro in a stepwise manner.
Fourteen meningococcal strains were selected towards rifampicin resistance in a stepwise manner in vitro; final MICs were between 8 and >256 microg ml(-1). Sequence analysis of a 295 bp subgenic fragment of the RNA polymerase beta-subunit (rpoB) gene from the original and the fully resistant strains revealed that, with one exception, the strain pairs differed by just one position in the deduced...
